Request for Financial Assistance

This form will be used to determine eligibility for financial assistance with home weatherization or electrification. Program funds can only be used to assist with heat pump installations; insulation and air sealing. Additionally, only homeowners deemed low-income or moderate-income are eligible for financial assistance.


The Energy Navigator may ask for documentation to verify the information submitted in this form.
